Self Referral FAQs

What is the process after I self-refer?

We will invite you to attend an initial chat and assessment to discuss what you would like us to look at. It might be that you have come to see us for a second opinion, or perhaps your own dentist doesn’t offer the service you require. We will provide a breakdown of our findings and give you a transparent answer as to whether or not we can help. If you would like to return to your own dentist to continue a suggested treatment, we will send over your notes, digital scans, X-rays, and any other clinical information to your dentist.

I’m an NHS patient – can I self-refer to a private dentist?

Yes. You may wish to speak to your NHS dentist before booking your consultation with us here at Hazelwood to keep them up-to-date.

Private Referral

You can refer to us privately for Implants, Invisalign®, Oral Surgery, CBCT, OPGs and Endodontics. Please click below to complete or download our online referral form.

Our referral packs are available from the practice if you prefer. All treatment charges will be thoroughly discussed with the patient, and written estimates will be given.

We are a dedicated referral centre for dentists for treatments such as Dental Implants, Extractions and Root Canals.

Our dentists will work closely with you to make sure your patient receives excellent care, informed by your clinical understanding of your patient’s dental and oral hygiene status.

We take great pride in looking after referred patients and will provide them with nothing but the highest level of care.

Oral Surgery

We are pleased to inform you that our professional dental surgeons at Hazelwood Dental Practice have been selected as providers for Level 2 endodontic and minor oral surgery.

Referring a patient for an extraction to us is straightforward. We can accept direct referrals. Simply complete our referral form and send it via secure email to our NHS address nhsdental.nlondon.mossolutionsV03447@nhs.net.

Referrals for endodontic treatment must be completed electronically using the form below.

This form should be sent to uclh.referrals.endodontics@nhs.net, who will, if appropriate, send it to us after they have assessed it for suitability.
